East Berkshire CCG is committed to supporting patients and their families if they are facing the end of their life
We are working on a five-year plan to make care at the end of life better and more responsive to the wishes of patients, their families and carers.
We have commissioned a new service which helps people with people with life-limiting illnesses manage their condition at home at any time of day or night.
Launched by Thames Hospice in April 2017, the community service includes a palliative care telephone service for people in Berkshire, giving advice to patients who are approaching the end of their lives and their families, as well as healthcare professionals who need guidance and support on delivering palliative care.
The specialist team is available 24/7, 365 days a year.
